Gynecomastia and Bodybuilding

A young bodybuilder came to see me, worried and concerned about relating a very typical story. Following a cycle of anabolic steroids, he began to experience a tenderness around his nipples. The areas soon began to swell and become even more painful. Soon he was embarrassed to take off his shirt and show off the physique that he had worked so hard to build. The more he dieted and built his chest, the worse the problem looked.

Studies have shown that the natural occurrence of the condition is between 32 and 65 percent. It results from the fluctuation of testosterone during puberty. While several drugs such as marijuana, medications for high blood pressure and ginseng have been implicated, certain anabolic steroids, such as Anadrol and testosterone, are the leading cause. These particular steroids undergo a process called aromatization in which the configuration of the male steroid molecule changes to estradiol, a feminizing hormone. Those who have receptor sensitivity will begin to develop breasts. So-called blocking agents like Nolvadex will not reverse the process once it has begun.

Fortunately the condition can be corrected by surgery. Being a master’s level competitive bodybuilder as well as a plastic surgeon, I realize how important it is to appear normal after the operation without any noticeable scars or deformities. I have developed corrective techniques that leave small, unnoticeable scars and allow the patient to quickly resume training. The chest should look normal both at rest and while posing.

The procedure is done in my outpatient surgery unit. The glandular tissue is removed and liposuction is used to contour the chest and increase definition. The patient wears a special elastic vest for several weeks to prevent swelling and control the healing process. Some exercise can be started in two weeks, with a full return to training in four to six weeks.

At present I have performed more of these types of procedures than any other surgeon worldwide.
